I've been told that 90 day reports at Patong are sometimes accepted but sometimes told to go to Phuket Town.
If this is true, I'll go direct to Phuket Town.

Latest reports are that the Patong Office is open again, but only as an information desk and that one must go to Phuket Town to actually do anything, including 90 day reports.

You must go to Phuket Town in order to process your 90 day report.
The following is what i provided for my last report in Nov., 2011. - TM47 - Copy of passport Face Page - Copy of a document verifying your address (lease, utility bill, telephone bill, cable bill, etc.) - prior 90-day reporting receipt - departure card - copy of latest Permission to Stay stamp |
