On the back page where it says "My previous visa for Thailand", do I put my last extension of stay info. or non-immigrant ( there is no info. I have a new passport and it just shows that it has been transferred ). Also, where it says where "I arrived", do I put the original date (prior to the extension of stay), or the last entry into Thailand?
Can I leave this part blank if I don't know it, or should I estimate the date if I'm getting it the same time as my extension of stay at a immigration office?
My current permission to stay, including re-entry permit, ends in October. Is there a way to change that (move it up) to December? The reason is that I will be out of the country in October and November.
