The address above is an old one. Below is the address and phone number as of 2010.
The very friendly old Japanese couple have retired.
The Consulate is still operated by the couples son.
You can down load the visa form off the internet. I would suggest the best idea is to call them to see what you need to send to them along with your passport.
As you do not reside in Honolulu I would also suggest contacting the consulate in Portland Oregon. Is is much more professional operation.
