As a general approach, opt for the least invasive procedure.
At 54 a hip replacement should not be the option so good that the ortho advised an alternative.
This again is less invasive than the re-surfacing and may be a good option at this time.
Given the limitations of this kind of forum for such medical advice, I would suggest a second opinion in a larger center.

Chronic renal failure is often associated with lowered protein levels in the plasma; this may lead to swelling of limbs.
Best to have a full blood protein test as well as liver functions and a complete blood count as anaemia is also associated with this condition.

If the pain is at the back of the top of the shoulder, it is probably a strain of the long head of the triceps muscle (search triceps muscle strain) and see if your symptoms fit.
Best to rest it with an ice pack over the painful area. Anti-inflammatory drugs may help as well.
