If you go to a big hospital or the police hospital they will do all the tests as specified. But in most small clinics they'll give you the documents you need for 100B or 200B and they will not do any test besides testing your blood-pressure and weight. If the doctor has a registration number, you'll be ok, and there's no need to go to an expensive hospital. At least, it was like this 3 years ago.
Make sure your doctor writes all the items/checks as specified by the labor department on his/her letter (some items are about very rare diseases like elephantiasis). I prepared the letter myself based on information from the labor department and a model I could find on the internet and let the doctor sign it. Requirements may change from time to time so be sure you've the latest information.

Thanks to everyone that replied to this thread. Just wanted to say:, "I am over it" 
I drove 40000 Km last year and lost 100 B to the police, despite my Thai driving style. So, if you think about it's actually a very good deal and it doesn't matter much that I didn't do anything wrong at the moment I was stopped. There must be numerous occasions I made serious traffic violations in front of the Thai police and they didn't stop me, while in my country they would have send the fine to my home address by mail.
In my country I get about average 8000B fines on 40000k, but I never felt angry about this because I deserved every single fine. Well, maybe I was a bit angry, but on myself.

Don't talk about drinking. I ever got robbed, called the police and they needed 2 hours to come from their office 1 km down the road. Then said they would bring me home in their car. The police officer was very drunk and smelled worse that a hospital. Sitting with them in the same car was much more scary than getting robbed.

Another remark I want to make: they stopped me on May 1st, which is an official holiday. Could it be possible that these officers were making some extra money outside working hours?
